We review our services regularly to check efficiency and quality. We need your opinion and ideas. We have a Patient Participation Group and welcome anyone who wishes to offer their time to give us their opinions to guide us in decisions about new services.
Click here for our PPG Report & Action Plan 2014-15
As well as our Patient Participation Group, we have a “virtual” patient group so you can have your say. We will ask you some questions from time to time, such as what you think about our opening times or the quality of the care or service you received. We will contact you by email and keep our surveys short so it shouldn’t take up too much of your time.
If you are happy for us to contact you occasionally by email please click the link below to open the sign-up form and complete all the fields.

If you prefer, you can download the sign up form as a pdf document, print it out, complete it and return it to the practice.

PPG Engagement and Information to share with Patients
Diabetes Video Opportunity for Patients
We are looking for people to help us make videos to support people with diabetes in North West London. Whether you have ideas about content, creative style, or whether you or someone you know would like to be featured in the video yourselves, we want to hear from you.
The number of patients with diabetes is increasing nationally and locally. Currently, almost 130,000 North West London residents have diabetes - that's over 6% of the population. With this in mind, Hammersmith & Fulham Clinical Commissioning Group is leading a programme of work to improve diabetes and pre-diabetes care across North West London.
We will be developing some videos to help people with diabetes manage their condition better. We are keen to work with you to identify:
· Whether you, or anyone you know, would like to be in a video about diabetes?
· What should the videos be about? What should our core message/s be?
· How can we best use video to educate and motivate people with diabetes to manage their condition?
We are holding workshops at St Paul’s Centre, Queen Caroline St, London W6 9PJ on:
· 30th June, 9am – 11am
· 5th July, 6pm – 8pm
If you would be interested in attending the workshops, getting involved or learning more about the project, please contact Adam Foster (Project Support Officer) on 0203 350 4054 or via email at Adam.Foster@nw.london.nhs.uk
There is a limited number of spaces at these workshops so please do get back to us as soon as possible if you would like to attend.
Choosing Wisely – Changing the way we prescribe
You should have received a stock of Choosing Wisely leaflets and posters for display in your waiting rooms and reception areas. Please can you send the information below to your PPG members or include in your patient newsletters
Choosing wisely: changing the way we prescribe
Last year the NHS in North West London spent £15 million on products that could easily be bought on the high street, often at very low cost. The NHS in NWL believes that it would make sense for most people to buy these products at the chemist or supermarket, to save NHS funds. They have put together three proposals on the theme of “changing the way we prescribe”.
See our poster and leaflet for more information.
Please visit https://choosingwiselynwlondon.commonplace.is and have your say. We really want to hear your views before the decision is made.
Carers Survey – Message from Ealing Local Authority
Ealing are developing a new Strategy for Carers in Ealing. We already have a lot of information from the national survey, but we would like to find out a bit more about what would make a difference to Carers lives.
The answers will help us make the future better for Carers. As an added incentive, if contact details are provided at the end of this survey, this will be entered in a prize draw for a £25 Marks & Spencer voucher.
You can access the survey via the following link: https://www.surveymonkey.co.uk/r/ealingcarer
