New Patient Registration

If you would like to register as a patient with Ealing Park Health Centre, please visit the practice and pick up a patient registration form or print the Registration form & New patient health questionnaire. Once you have completed the forms, you will need to attend the Surgery to provide proof of identity and proof of address within our catchment area to complete the registration process.

Proof of identity accepted:

  • Passport
  • Driving licence
  • Identity Cards

Proof of address accepted:

  • Utility Bill
  • Council Tax Bill
  • Tenancy Agreement

Practice Area

The practice has a specific registration area, a detailed registration area map is shown below. We will register any patient who lives in this area, you may be asked for proof of address. If you move but still live within our practice area we can continue to be your GP.

If you change your name, address or phone number please let reception know as soon as possible. If you move outside our practice area you will have to register with another surgery.
